What to bring to the sixth appointment.
What to bring to a sixth appointment is a dated account a clinician can read in minutes: what hurt, where, how badly, for how long, what you took and whether it helped, plus the questions this visit is for. Endonem compiles it as a sealed report with a prep pack of up to three questions built from your own record.
